Your Health Care Team
Many people are involved in your care during your hospitalization. Here are some of the health care providers you’ll most likely see during your stay – they are all part of your health care team. You also are a member of the team. Your active involvement is a vital part of the healing process.

Care Coordinators: Care coordinators are registered nurses who coordinate the efforts of the health care team to give you the best care possible. Care coordinators promote communication between the patient/family and health care team, address questions or concerns, including those with your insurance company, and plan for your needs at discharge.

Medical Staff: Your attending or staff physician heads your health care team, overseeing all aspects of your care. Although you may not see this person every day, he or she is in daily consultation with other staff members, coordinating their efforts. These other staff members include fellows, chief residents, other residents and professional staff.

Nursing Staff: Nurses are responsible for monitoring your care, planning and coordinating your daily care with other health team members and providing specialized treatment and care.

Social Work and Discharge Planning Staff: Social workers and nurses skilled in home care, community placement and support services are available on each patient care unit to help you and your family cope with issues that can accompany illness and to coordinate care you may need after you’ve left the hospital.

Chaplains: A chaplain is assigned to most patient care units to provide spiritual, ethical and religious counsel and support.

Other Health Care Professionals: During your stay, you may come into contact with many other health care professionals, including radiology technologists, therapists, pharmacists, laboratory technicians and dietitians, as well as many other important members of your health care team.

Students: An important responsibility of the medical center is training medical, nursing and other health care students through their direct involvement in patient care. All students are under the close supervision of staff members.
