Pamela Phelps, Pharm.D., FASHP

Director of Clinical Pharmacy Services Residency Program
Director Clinical Associate Professor, College of Pharmacy

Pam graduated with a B.S. in Pharmacy in 1982 and a Pharm.D. in 1987. Her background includes experiences as an ICU pharmacist, drug use evaluation specialist, clinical supervisor, and pharmacy specialist in critical care. She is Director of Clinical Pharmacy Services for Fairview Hospitals, which encompasses Drug Policy, Formulary, Clinical Service provision, and the Residency Program. Pam is a Past President of the Minnesota Society of Health-System Pharmacists, is past President of the Central Minnesota Society of Health-System Pharmacists, is a member of the UHC Research and Education Council, and has served on ASHP's Council for Educational Affairs. Pam's practice interests include critical care, pain management and infectious disease. Pam's management interests include clinical management, quality improvement, and medication safety.

Scott Knoer, M.S., Pharm.D.

Director of Pharmacy, University Campus

Scott Knoer is the Director of Pharmacy at the University of Minnesota Medical Center, University Campus. Scott received his Pharm.D. at the University of Nebraska College of Pharmacy. He completed a combined two-year residency in pharmacy practice and Master's degree in Hospital Pharmacy at the University of Kansas Medical Center. Scott's focus is on creating failsafe medication systems and advancing the practice of pharmacy. Scott is passionate about developing pharmacy leaders. Scott is a Board Member of the ASHP Section of Practice Managers.

John Pastor, III, Pharm.D.

John earned his undergraduate and Doctor of Pharmacy degrees from the University of Minnesota, and completed an ASHP-accredited Pharmacy Practice Residency at the University of Minnesota Hospital and Clinic. John’s practice interests include infectious diseases, pediatrics and safe medication delivery through process standardization. John is actively involved in local, state and national pharmacy organizations and is a past President of the Minnesota Society of Health-System Pharmacists.

Diane Ehlers, Pharm.D.

Assistant Director, Pharmacy Operations

Diane Ehlers received her Doctor of Pharmacy degree from the University of Minnesota. Diane completed an ASHP-accredited Pharmacy Practice Residency at the University of Minnesota Medical Center. Her operational interests include the optimization of medication preparation and distribution, incorporation of automated medication delivery systems, and implementation of process improvement strategies.

Joseph B. Stanek, Pharm.D.

Director of Pharmacy, Riverside Campus

Joseph is the Director of Pharmacy at the University of Minnesota Medical Center, Riverside Campus. Joseph received his Bachelor of Science and Pharm.D. from the Midwestern University-Chicago College of Pharmacy. He worked as a clinical pharmacist then became Pharmacy Director at Adventist Health System before coming to the University of Minnesota Medical Center. Joseph serves as the preceptor for the operations management rotation on the Riverside campus.

Kelly Ferkul, Pharm.D., BCPS

Clinical Pharmacy Manager, Riverside Campus

Kelly received her Pharm.D. degree from the University of Minnesota, College of Pharmacy in 1999. She completed a pharmacy practice residency at Fairview-University Medical Center in 2000. Before becoming a manager in 2003, she practiced in both decentralized pharmacist and clinical specialist roles. Her areas of interest include general medicine and diabetes management.

Dave Moss, Pharm.D.

Clinical Pharmacy Manager, Fairview Home Infusion

Dave received his B.S. degree in Pharmacy in 1986 and graduated with his Pharm. D. degree from the University of Minnesota in 2004. His background experiences include NICU / PICU Satellite pharmacist and home infusion clinical pharmacist. He currently is the clinical pharmacy manager for Fairview Home Infusion. Dave’s practice interests include management of the critically ill patient in the home and infectious disease. Dave’s management interests include clinical management of staff members, patient medication safety and compliance.

Joshua Breeding, Pharm.D., BCPS

Clinical Specialist, Cardiology/Pulmonology

Josh received his B.A. degree in biochemistry from Gustavus Adolphus College and went on to receive his Pharm.D. degree from the University of Minnesota. He then completed an ASHP-accredited pharmacy practice residency at the University of Minnesota Medical Center. Josh’s areas of interest include heart failure, pulmonary hypertension and anticoagulation. He serves as the primary preceptor for pharmacy practice residents and acute care pharmacy students. Josh is Team Leader for the cardiology area.

Katie A. Carls, Pharm.D., BCPP

Clinical Specialist, Psychiatry
Clinical Assistant Professor, College of Pharmacy

Katie received her B.S. and Pharm.D. (2001) from North Dakota State University, College of Pharmacy. She completed a general practice residency at Mayo Medical Center, Rochester, Minnesota in 2002. In 2003, she completed a psychiatry specialty residency at UMKC-Western Missouri Mental Health Center in Kansas City, Missouri. Katie has been with the University of Minnesota Medical Center since 2003 and is currently a psychiatric clinical specialist.

Melissa Carlson, Pharm.D.

Clinical Specialist, NICU

Melissa Carlson obtained her Doctor of Pharmacy degree from the University of Minnesota in 1999 and completed an ASHP-accredited pharmacy practice residency at Fairview-University Medical Center (now University of Minnesota Medical Center, Fairview). Her pharmacy experience includes positions as a Decentralized Pharmacist in adult internal medicine/cardiology and pediatrics and a Clinical Specialist in general pediatrics/pediatric oncology. Melissa is currently the Team Leader for the Pediatrics area. In addition to her daily patient care activities, she serves as a preceptor to pharmacy students and residents. She has been active in the Vermont-Oxford Collaborative, NICU Collaborative Practice Council, and the Safest in America Pediatric Collaborative.

Virginia Ghafoor, Pharm.D.

Clinical Specialist, Pain Management

Virginia graduated from the University of Nebraska College of Pharmacy in 1991. She completed an Oncology Research Fellowship at the University of Wisconsin in Madison. Dr. Ghafoor spent the next four years as an Assistant Professor of Pharmacy at the University of Rhode Island and Pain Pharmacotherapy Research Investigator at the Interventional Pain Management Clinic in Providence, RI. Ginnie's current position is as a clinical pharmacy specialist in pain management. Her particular interests are narcotic over-sedation, benzodiazepine use, and inpatient pain management.

Robert Haight, Pharm.D., BCPP

Clinical Specialist, Psychiatry
Clinical Assistant Professor, College of Pharmacy

Robert graduated with a Pharm.D. degree from the University of Minnesota College of Pharmacy in 1998. He completed a Psychopharmacology Research Residency at Regions Hospital, Saint Paul, MN in 1999. He then became Psychiatry Specialist for Advance PCS, a managed care company in Bloomington, MN. Bob came to Fairview-University Medical Center in 2001 as a Psychiatry Clinical Specialist.

Jennifer Jerabek, Pharm.D.

Clinical Specialist, Pediatrics

Jennifer received her Doctor of Pharmacy degree from the University of Minnesota in May 2002. Following graduation, she did a one-year pediatric specialty residency at The Children's Hospital in Denver, Colorado. She currently rotates between being a clinical specialist on the pediatric floors and being a decentral pharmacist. Jennifer also is a member of the Pediatric Pharmacy Advocacy Group (PPAG). Jennifer loves the challenges in working with pediatric patients and enjoys the fulfillment she receives each day.

Melissa Kennedy, Pharm.D., BCPS

Clinical Specialist, Solid Organ Transplant
Clinical Assistant Professor, College of Pharmacy

Melissa received a B.S. degree in pharmacy from the University of Iowa. She completed her Pharm.D. at the University of Minnesota, and an ASHP accredited residency in pharmacy practice at The University of Minnesota Hospital and Clinic. Melissa is a preceptor for the solid organ transplant rotation for the Pharm.D. students and residents. Her areas of interest include immunosuppressive medications and infectious disease.

Janet Kubly, Pharm.D.

Clinical Specialist, Pediatrics

Janet is one of the Pediatric Clinical Specialists for the Children's Center and also works as a decentralized pharmacist at Fairview-University Medical Center. Janet received her Pharm.D. degree from South Dakota State University in 2000. She completed a general pharmacy practice residency at Fairview-University Medical Center in 2001. Janet enjoys working in pediatrics because of its many unique challenges. She is also inspired by the patients' resilience and ability to adapt.

Bruce Lohr, Pharm.D., BCPS

Clinical Specialist, Cardiology and Surgical Intensive Care
Clinical Associate Professor, College of Pharmacy

Bruce received his B.S. and Pharm.D. degrees from the University of Minnesota College of Pharmacy. He also completed a one year clinical residency at The University Hospital and Clinic. Bruce has been at the University of Minnesota for nineteen years as a clinical specialist. He serves as co-coordinator for the University of Minnesota College of Pharmacy Pharm.D. acute care students. Bruce is currently Team Leader for the critical care areas. His areas of interest include infectious disease, organ transplantation, and cardiovascular drug therapy.

Lisa Lohr, Pharm.D., BCPS

Clinical Specialist, Oncology
Clinical Assistant Professor, College of Pharmacy
Pharmacy Practice Residency Coordinator

After receiving her B.S. degree in pharmacy from the University Iowa in 1986, Lisa completed a two-year combined general clinical residency and Pharm.D. degree program at the University of Washington in 1988. Lisa has practiced in the oncology area, concentrating on the supportive care of the cancer patient, chemotherapy administration and avoidance of adverse effects, and anticoagulation. In addition, Lisa serves as the coordinator for the pharmacy practice residency program. Lisa is currently Team Leader for the oncology and med/surg area.

Darlette G. Luke, R.Ph.

 Manager, Investigational Drug Services

Darlette received her B.S. degree in Pharmacy from the University of Minnesota in 1985. She currently manages Fairview IDS Pharmacy. IDS Pharmacy is responsible for coordinating the study medication for all inpatient research protocols as well as dispense study medication for outpatient protocols. This requires interaction with the principal investigator, nurse coordinators, drug manufacturers, and governmental regulatory agencies. University of Minnesota Medical Center is participating in research protocols at all four phase levels.

Richard Mandt, Pharm.D.

Clinical Specialist, Medical Intensive Care
Clinical Assistant Professor, College of Pharmacy

Dick received his B.S. and Pharm.D. degrees from the University of Minnesota. He has practiced in both staff and clinical roles. Dick has been with Fairview University - Riverside since 1980 and has held a variety of clinical and management positions. He is a Past President of the Central Minnesota Society of Health-System Pharmacists. In addition to critical care pharmacotherapy, Dick's areas of interest include nutrition support, infectious diseases and clinical pharmacokinetics. He also is interested in the system of care and its relationship to patient safety.

Kelly McMonigal, Pharm.D., BCPS

Clinical Specialist, Medicine and Surgery

Kelly received her Pharm.D. degree from the University of Minnesota, College of Pharmacy in 1999. She completed a pharmacy practice residency at Fairview-University Medical Center in 2000. She then worked as a decentralized pharmacist for three years, before becoming a specialist. Kelly is Team Leader for the internal medicine and solid organ transplantation areas.

John Rogosheske, Pharm.D.

Clinical Specialist, Adult Bone Marrow Transplant
Clinical Assistant Professor, College of Pharmacy

John received his B.S. and Pharm.D. degrees from the University of Minnesota in 1986. He has worked in the area of hematopoetic stem cell transplant for over 15 years. Previously, John practiced as a clinical pharmacist in pediatrics and adult intensive care. His areas of interest and research include infectious disease, oncology, nutrition support, and anti-emetics.

Nancy Rydholm, Pharm.D.

Clinical Specialist, Pediatric Bone Marrow Transplant
Clinical Assistant Professor, College of Pharmacy

Nancy received her B.S. degree from North Dakota State University and her Pharm.D. from the University of Nebraska Medical Center. She did a one year general residency with clinical emphasis at the V.A. Medical Center in Lincoln, NE. She has been a clinical pharmacist at The University of Minnesota Hospital and Clinic since 1986. Nancy's areas of interest include bone marrow transplantation, pediatrics, oncology and nutrition support. She has been active in MSHP and CMSHP and is currently the treasurer of MSHP.

Angela Skoglund, Pharm.D.

Clinical Specialist, Pediatrics

Angie received her Doctor of Pharmacy degree from the University of Minnesota College of Pharmacy in May, 2000. While completing a Pharmacy Practice Residency at Fairview-University Medical Center, she developed a strong interest in pediatrics. Angie continues to work at University of Minnesota Medical Center, alternating weeks as a decentralized pharmacist and as a clinical specialist in the Children's Center. She serves as one of the preceptors for the general pediatric rotation for Pharm.D. students and residents.

Katie Tu, Pharm.D.

Clinical Pharmacist, Pediatrics

Katie received her Doctor of Pharmacy degree from the University of Iowa in May 2001. While attending pharmacy school, Katie worked as a pharmacy intern at the University of Iowa Hospitals and Clinics and developed a strong interest in hospital pharmacy practice. Katie completed a one-year hospital residency at FUMC where she evaluated the insulin administration practice and developed an intensive insulin protocol for patients in the SICU. Katie's areas of interest include pediatrics, cardiology, and critical care. Katie currently works in the Children’s Center alternating between decentral and clinical roles.
